ALL-AMERICAN KIDS AWARDS
NOMINATION FORM

Bank of America and the Quincy Society of Fine Arts want you to help us find three ALL-AMERICAN KIDS. We would like your nominations for young people up to and including age 14. Submit nominations by completing the form below. Deadline for accepting entries for the 2008 All-American Kids Awards is Friday, January 18, 2008.

ARTS/HUMANITIES: An award for a student with exceptional skill in the language, visual or performing arts. The winner may be distinguished in the disciplines of history or literature.

MATHEMATICAL/NATURAL SCIENCE: An award for a student whose curiosity has led to outstanding achievement in the exploration and discovery of the environment.

SPORTS: An award for a student who has been challenged by athletics and turned dreams and goals into concrete reality.
